We are looking for an enthusiastic and highly motivated individual to join our award winning social prescribing service - Live Well Wakefield. The service provides information, support, and advice to adults aged 18+, helping them improve their health, wellbeing, and independence.
You will act as principal point of contact/liaison for clients, members of the public, and other professionals, taking and processing referrals and enquiries by means of face to face, telephone, and electronic contact.
It is essential you have experience of working within a team setting, and in an environment dealing with members of the public. You will also be required to possess good keyboard and IT skills including working knowledge of Microsoft Office, and the ability to prioritise tasks and work under their own initiative. Previous experience of working in a health or social care setting, and of using SystmOne would be advantageous.
You will communicate frequently with colleagues, clients, community anchors, primary care networks, housing associations and other professionals from across the Wakefield District and are therefore required to be a confident and natural communicator with excellent organisational and problem solving skills, to coordinate the multiple aspects of this job role.
The post is temporary initially until December 2022 with the possibility of extension to March 2023.
