An exciting opportunity has arisen to join our award-winning social prescribing service as a Social Prescribing Link Worker. The role involves supporting adults aged 18+ to identify what matters to them and helping them to explore available services and provision to improve their health and wellbeing.
Social prescribing empowers people to take control of their health and wellbeing through referral to link workers, who use a holistic approach to connect them to non-medical services. You will need a can-do attitude, and a drive to support others by connecting people to support, often in their local communities.
You will work closely with GP surgeries across a primary care network to grow social prescribing within general practice and empower clients to access non-medical, social solutions to address their needs; in turn working to reduce attendances at general practice, prevent hospital admissions and providing an alternative to medication.
You will work alongside a highly skilled and motivated team to deliver a well-respected, high quality and personalised referral based service across the Wakefield District.
You will have a good understanding of health and social care and of the community and voluntary sectors. You will understand the influence these services can have on improving a client's health and wellbeing, and possess the skills to create rapport, influence positively, and support clients to identify and achieve personal goals.
You will work closely with a wide range of health, social care & VCSE sector organisations and therefore will be required to be a confident and natural communicator with excellent organisational skills to coordinate the multiple aspects of this job role.
The post is initially temporary to March 2023, with the possibility of extension to March 2024.
