What does a Social Prescribing Advisor do?

Advisors at Live Well Wakefield work with people at risk of reduced wellbeing and independence, providing them with social prescribing information, advice and support on a 1:1 basis. This role will focus on a new project supporting people to access services and address social needs that will help them to enter into, and/or sustain employment, staying well whilst in work.

The role is in a multi-disciplinary environment, and includes developing community resources and networks between all sectors to improve wellbeing and reduce inequalities for local people.
