As a Social Prescribing Advisor, you will support vulnerable adults across the Wakefield district to improve wellbeing, independence, and access to employment opportunities. Working 1:1 with clients, you’ll assess individual needs, develop personalised wellbeing plans, and connect people with local services and activities that can enhance their quality of life.
You’ll manage a busy caseload, empowering individuals to make informed decisions, reduce barriers, and build confidence. This role works across sectors, health, social care, VCSE, and employers, to strengthen community links, reduce inequalities, and improve outcomes around economic inactivity.
Key responsibilities include liaising with partners, delivering wellbeing information sessions, advocating for clients, and contributing to the development of local provision. You’ll maintain accurate records, support service improvement, and stay up to date with local opportunities and national developments in social prescribing.

Nova and South West Yorkshire Partnership NHS Foundation Trust deliver Live Well Wakefield, which is commissioned by Wakefield Council and Wakefield’s seven Primary Care Networks.
